AUTHORISATION ISSUED BY A REGSTERED DEALER TO A TRAVELLING SALESMAN/REPRESENTATIVE

3. Name and address of the travelling salesman/representative

4. Specimen signature of the travelling salesman/representative

5. Period for which the authorisation is valid

Note : 1. The authorisation shall be invalid if any of the columns are left unfilled.

2. The person signing the authorisation shall be the same as that shown in Column 2 of the permit in Form 7.
