I hereby give you notice to attend in person or through an authorised representative and show cause why a penalty not exceeding rupees one thousand should not be imposed upon you under sub-section (6) of section 5.

(b) Whereas I am not satisfied that the return furnished by you for the month / period ...................... is / are correct and complete:
